- Initial full health consultation $250
- Follow up office visits hourly $200 (½ hour $100)
- Phone consultations $25 up to 15 min, $50 for 30-60 min
- Prescription Fee (without visit) $50
- Appointment cancellation less than 24 hours $50
- Lab work order and phone review (if separate from appt) $75
